亚甲基四氢叶酸还原酶A1298C基因多态性与肝细胞性肝癌遗传易感性的Meta分析

Meta-analysis of the association between MTHFR A1298C polymorphism and susceptibility to hepatocellular carcinoma

摘要: 目的 探讨亚甲基四氢叶酸还原酶(methylenetetrahy drofolate reductase,MTHFR)A1298C基因多态性与肝细胞性肝癌(hepatocellular carcinoma,HCC)遗传易感性的关系。方法 计算机检索PubMed、EMbase和中国学术期刊全文数据库等,收集有关MTHFR基因多态性与HCC遗传易感性的病例-对照研究资料,按照纳入标准选择文献,提取相关数据进行Meta分析。以病例组和对照组基因型分布的比值比(OR)为效应指标,对纳入文献进行异质性检验,应用Stata 12.0软件对各研究原始数据进行Meta合并,计算合并效应量OR值及95%可信区间(95%CI)。结果 共纳入6项病例-对照研究,病例组为1 708例,对照组为2 911名。等位基因比较模型(C vs A):OR(95%CI)=0.95(0.85~1.06),P=0.342;纯合子比较模型(CC vs AA):OR(95%CI)=0.55(0.38~0.79),P=0.001;隐性模型(CC vs AC/AA):OR(95%CI)= 0.54(0.38~0.78),P= 0.001。结论 MTHFR A1298C 基因多态位点CC基因型能够降低HCC遗传易感性,为保护基因型。

Abstract: Objective To explore the relationship between the methylenetetrahy drofolate reductase(MTHFR) A1298C polymorphism and susceptibility to hepatocellular carcinoma(HCC). Methods A preformulated search strategy was used to search the English-and Chinese-language research literature on the association between the MTHFR A1298C polymorphism and HCC.The strength of the association was assessed in terms of odds ratios(ORs) and 95% confidence intervals(CIs),and heterogeneity was evaluated.All analyses were conducted using Stata 12.0. Results A total of six case-control studies involving 1 708 cases and 2 911 controls met the inclusion criteria.Significantly reduced risk of HCC was found in the homozygote comparison(CC vs AA:OR 0.55,95%CI 0.38~0.79,P=0.001) and recessive model(CC vs AC/AA:OR 0.54,95%CI 0.38~0.78,P=0.001).However,no significant increase or decrease in risk was found in the additive model(C vs A:OR 0.95,95%CI 0.85~1.06,P>0.05).No publication bias was found using any of the genetic models. Conclusions Genotype CC of MTHFR A1298C may decrease the risk of HCC and may therefore constitute a protective factor for HCC.
